Please complete a separate form for EACH CHILD you wish to PRE-Register (Wait List) for Nippers at Mount Martha Life Saving Club (MMLSC) for season 2015/2016.
Pre-Registration is season specific and limited to children who meet age requirements for the upcoming season ie. only complete pre-registration form for children aged 5 years to 13 years as at 30th September 2015.
Note: Some information will be carried across from the previous entry if completing for more than one child, so if it needs to change please do that manually.
Important Note: It is club policy if one child from a family is offered a Nippers place priority will be given to any other siblings pre-registered in other age groups.
Therefore please make sure you tick the Multiple SiblingsĀ box below if you are pre-registering more than one sibling so that family groups can be recognised.
